NC-4EZ Form  (All new and former employees must submit this NC form)

Form I-9  (only needed for NEW employees)

Form W-4  (ALL new and former basketball scorekeepers and volleyball referees must complete)
Form W-9 (for adult baseball umpires) or Form W-4 (for student baseball umpires)

If the employee is a new hire, i.e., has never worked for SIAA prior to current season:
Form W-4 (but W-9 for adult baseball umpires)
Form NC-4EZ
Form I-9 

If the employee has worked for SIAA in a prior year, but not in the current calendar year:
Form W-4
Form NC-4EZ 

If the employee has worked for SIAA in the current calendar year:
No forms needed, unless the employee needs to update a name, address, or withholding allowances.
If an update is needed, complete Form W-4 and NC-4EZ.
